Physicians may request a Certificate of Professional Conduct (CPC) in one of two ways:
- If you are an active member with the College, and currently have a licence, please log in to your member portal to request your CPC. The option to request a CPC will be available on your home page. - If you are no longer a member of the College, please email This email address is being protected from spambots. You need JavaScript enabled to view it. to request your certificate. Once an agent receives the request, an invoice will be issued with instructions on how to pay.
A payment of $100 will be required by cheque or etransfer before your CPC is issued.
